Care Check's on line disclosure service is available for companies who are registered with us.The on line form cuts down on the mistakes made filling out the form and limits the need for applications to be sent back for amendment. In order to use it you will need to obtain a user name and password from Care Check. The form will be printed out and sent to the employer for the signature of consent and so the identification documents can be verified.
